การเตรียมตัวและการดูแลของการผ่าตัดริดสีดวง

การเตรียมตัวก่อนการผ่าตัด งดน้ำงดอาหารอย่างน้อย 8 ชั่วโมง มาเตรียมตัวล่วงหน้าตามเวลาที่ทางรพ.แจ้งเพื่อทำการเจาะเลือด, x-ray, +/ - ทำคลื่นไฟฟ้า เพื่อเตรียมความพร้อมสำหรับการผ่าตัด ช่วงระหว่างการผ่าตัด วิสัญญีแพทย์จะทำการบล็อกหลัง หรือฉีดยาให้หลับตามที่กล่าวมาข้างต้น ในระหว่างการผ่าตัดใช้เวลาประมาณ 30 นาที - 1 ชั่วโมง หลังผ่าตัดจะสังเกตอาการที่ห้องพักฟื้นจนมั่นใจ แล้วจึงกลับไปพักที่ห้องได้ การดูแลหลังการผ่าตัด นอนโรงพยาบาลหลังผ่าตัดเฉลี่ย 1-2 คืน หลังกลับบ้านแล้วสามารถใช้ชีวิตประจำวันได้ตามปกติ ไม่ควรออกกำลังกายหนักๆ เช่น วิ่ง ฟิตเนส เต้นแอโรบิค โยคะ ในช่วง 1 เดือนแรก จะมีการนัดติดตามอาการครั้งแรกประมาณ 1-2 สัปดาห์หลังผ่าตัด

ผ่าตัดริดสีดวงจำเป็นต้องบล็อกหลังจริงหรือ

• ในอดีตการผ่าตัดริดสีดวงจำเป็นต้องทำการบล็อกหลัง(spinal anesthesia) หรือการฉีดยาชาเข้าบริเวณช่องน้ำไขสันหลัง เพื่อระงับความปวดขณะผ่าตัด ซึ่งมีผู้ป่วยส่วนนึงที่กลัวหัตถการนี้มาก หรืออาจเกิดภาวะข้างเคียงเช่นปวดศีรษะ ปวดหลัง หรือปัสสาวะไม่ออกได้ • แต่ในปัจจุบันพบว่าการฉีดยาให้หลับร่วมกับการฉีดยาชาเฉพาะที่ (TIVA + perianal block) สามารถทำการผ่าตัดได้อย่างปลอดภัยเช่นกัน โดยวิธีนี้ไม่จำเป็นจำต้องนอนราบหลังผ่าตัด 6-8 ชั่วโมง ลดภาวะปัสสาวะไม่ออกหลังการผ่าตัดได้ จึงเป็นที่นิยมมากขึ้น สุดท้ายแล้วอาจต้องมีการปรึกษาพูดคุยกับทางวิสัญญีแพทย์ เพื่อพิจารณาวิธีที่เหมาะสมที่สุดสำหรับผู้ป่วยที่สุด

Preparation before surgery / during surgery / care after hemorrhoid surgery

Preparation before surgery

Cut food or drink for at least 8 hours.
Come prepare in advance according to the time notified by the hospital. To do blood draws, x-rays, +/ - electrocardiograms.
To prepare for surgery.

During surgery

The anesthesiologist will perform a dorsal block. Or give an injection to make you sleep. As mentioned above
During surgery it takes approximately 30 minutes -1 hour.
After surgery, symptoms will be observed in the recovery room until you are confident. Then you can go back to your room.

After surgery

Stay in the hospital after surgery is around 1-2 nights.
After returning home, Patient can continue your daily life as usual.
Do not do heavy exercise such as running, fitness, aerobic dancing, or yoga during the first month.
There will be a first follow-up appointment approximately. 1-2 weeks after surgery

Is it really necessary to have hemorrhoid surgery to spinal anesthesia?

• Nowadays it is found that injections to put you to sleep together with injections of local anesthesia (TIVA + perianal block) can be performed safely as well. With this method, there is no need to lie down for 6-8 hours after surgery, reducing the chance of incontinence after surgery. Therefore it is more popular. Ultimately, a consultation with an anesthesiologist may be required.
